What is the Disciple Maker Index Survey?

The Disciple Maker Index survey consists of a 75-questions that invite parishioners to reflect on where they are on their journey of discipleship and enables parish leadership to make data-driven decisions to help parishioners grow in their faith.

Survey administration is overseen by Catholic Leadership Institute in your parish who provides templates to support getting your parishioners involved.

Comprehensive and useful reports will be prepared to share your results, including an interactive software that allows you to dive deeper into responses of interest.

Context for your data will be made available so you can compare your results with national trends, other parishes in your diocese and the top three performing parishes in the country.

Faithful and expert consultation will be provided by CLI as they help you interpret survey results and create a plan to translate the data into action.

We will ask you to relect on your own spiritual growth and enable you to provide feedback on our parish's efforts to hel you grow.  All responses will be confidential, and the parish will only receive information abot the community as a whole.

We are trying to get the highet response rate possible.  This information will be invaluable to Father Dwight and our various ministries as we plan for the future and strive to be the best disciples we can be.  When we receive the results, we will share what we have learned with the entire parish.

Who Are We Asking to Take the Survey?

We need your help!  We are asking ALL St. Michael's parishioners to participate in the survey and we need 100% participation.

When Does the Survey Period Occur?

The survey begins on September 8, 2023, and closes on October 9, 2023.

How Do I Take the Survey?

The primary method of taking the survey is online, either via smartphone, tablet, or desktop computer.  The survey should take no longer than 10-15 minutes to fill out.

Finally, for those who need an alternative manual form of input, physical paper forms will be available in the Family Life Center hallway for filling out and submittting your survey input.
