In order that our parish can better serve your needs,
we ask that all parishioners register with the parish. This
can be accomplished by filling out a parish registration form and
submitting to the office. Forms can be picked up in the back
of the church, from the parish office, or printed out from this
website (see BELOW). After
filling out the form, submit it to the parish office in one of
several ways:
1. Bring it to the parish office. During
business hours, turn it in to a member of the
parish staff.
Outside of business hours, deliver it via the mail slot in the
parish
office door
off of the main hallway of the Family Life Center.
2. Bring the completed form to mass and put it
into the collection basket.
3. Mail the completed form to the parish office (
HERE for address).
4. Fax the completed form to the parish fax (
HERE for fax number).

The parish registration form
can be downloaded via the link below. It is delivered in Adobe
Acrobat format. In order to view and print out the form, your
workstation must have the Adobe Acrobat Reader, a free download from
Adobe. If you do not have the Reader, you may download it (at
no cost) from Adobe via the link below. If you have any
